PARTICIPANT REGISTRATION

Do you have or have you had any medical condition or disability that may affect your efficiency as a

participant, your safety or the safety of the public?

If YES, you will be required to submit a Medical Consent Form completed by a medical practitioner

before participating. RDAASW may in its reasonable discretion require you to provide a Medical

Consent Form completed by a medical practitioner even if you have declared you do not have/had

any medical condition or disability.

$75.

Registers the participant with RDA Australia and insurance policy.


$40 per session. Paid by the term in advance.

Includes a nationally accredited Coach, accredited horse/pony and one specially trained support person.


As arranged with your referring physiotherapist.


Our coaches and volunteers have been rostered on specifically for your session. For these reasons we

have a cancellation policy to compensate for our coach’s and volunteer’s time and lost income.


Cancellation by participant

Sessions must be cancelled in person, or via phone 9795 7487 or 0439 923 314. Please note that we do not monitor texts and email may not always be a reliable form of communication especially at peak busy periods. Please speak with us directly if you need to cancel.


If a participant does not show for their session with no direct verbal indication  to our team, RDAASW

reserves the right to charge the full session fee. No refund or credit will be given.


If a cancellation is made less than 24 hours  in advance of the session start time or on the day  of the session, RDAASW reserves the right to charge the full session fee. No refund or credit will be given.


If a participant arrives late for a session, the participant may be unable to join the session and RDAASW reserves the right to charge the full session fee. No refund or credit will be given.

Providing participants provide at least 24 hours-notice to RDAASW of a session cancellation, RDAASW will issue a refund or credit for the cancelled session.


Cancellation by RDAASW

RDAASW reserves the right to cancel a session for any reason, including but not limited to weather

conditions, safety, security, staffing levels or horse welfare. In this event participants will receive a

credit to the value of the cancelled session.

In the event a participant is refused a session by RDAASW for reasons including but not limited to lack of appropriate clothing, exceeding weight limit or inappropriate behaviour, RDAASW reserves the

right to charge the full session fee and no refund or credit will be given.


In the event RDAASW must cancel a session (including but not limited to weather), you will receive an email and/or text message. Although we will endeavour to receive confirmation from you, it is ultimately the participant’s responsibility to make other contact arrangements if you’re unable to receive emails and/or texts in the period leading up to your session.


I understood and acknowledge that this form is a registration of interest only and does not guarantee

an offer to participate in RDAASW programs. Offers to participate are based on the availability of

RDAASW resources, the support needs of the participant and the goals of the participant.


Where the applicant is under 18 years of age or cannot provide informed consent, this form must be signed by the applicant’s parent or legal guardian.

Personal information is collected for the purpose of assessing the participant for potential offer of session.
